Special Instructions for Completing NFA Applications
The application can be obtained at NETC Application for Admission (Long Form) - FEMA Form 119-25-1. Be sure and fill it out, paying particular attention to the block 16, since this is what decides acceptance or rejection. Put block 16 information on another sheet of paper with the name and SSN at the top and type SEE ATTACHMENT in the actual block 16. I'm attaching a sample block 16, don't use the exact wording, but "tweak" it for the individual and Department. Use the student selection criteria, the job descriptions and the course description I have included and tell how the individual will use it and how the Department will benefit from their attending. Be sure and include the individuals email address if they have one and then fax it to the number below. A majority of the NFA Regional delivery courses are considered as "end user" courses, therefore do not make any reference in the block 16 about being an instructor or teaching the course in their agency unless the course is instructor related.
Don't try to "enhance" their chances of acceptance by adding experience or duties from another organization. For example: USAR team experience should not be mentioned unless applying as a member of the USAR team and not a Department. There must be a relationship between the organization applying from and the course, not just experience from anywhere.
Also on all applications make sure the title on line 22b of the approving signature on line 22a is either the Chief or the Training Chief of the Department, not just any "Chief" from the Department. If required, include a letter of recommendation and an organizational chart.
Please send all applications to the address below. I will check them over and then forward them to the National Fire Academy for final processing. To allow adequate time for processing please submit the completed applications to me no later than 50 days prior to the scheduled starting day of the class.

SAMPLE BLOCK 16
As a (whatever, Lieutenant, Captain, Battalion Chief) who will have the responsibility of command at a variety of incidents in my (city, community, etc.) this course will provide me with the knowledge to assist me in decision making process while allowing me to recognize the strategic and tactical considerations necessary for the control and extinguishment of fires and control of incident scenes from residential house fires, to multi-occupancy and commercial properties. This course will provide me with a better understanding of problem solving, size-up, strategy, tactics and the development of an action plan. It will also provide me with an in-depth understanding of multi-company operations, building construction and incident organization. This course will allow me to focus more on firefighter safety on the incident scene, ensuring a safer, more organized incident scene.
